If your card is lost or stolen contact JPMorgan Chase at (800) 316-6056 and the Card Program Administrator at email@example.com. For identification purposes you may be required to provide the last four digits of your employee ID, mother’s maiden name, or date of birth.
Replacement T&E Cards can be mailed within seven days at no cost. Rush orders for replacement T&E Cards can be delivered within 24-48 and a rush fee may be charged on the cardholder’s statement.
