ISPs must be pre-approved by the ISP Steering Committee (comprised of individuals from Cost Analysis, Sponsored Projects Accounting, and Customer Accounts) as a service provider and authorized to bill other WashU departments for goods and services prior to providing such goods and/or services for the first time. Completion of this form will facilitate this review.
